Dear VIA Orthopedics Patient,

We regret to inform you that VIA Orthopedics, LLC will be permanently closing as of July 1, 2025.

 

It has been our privilege to care for you and/or your loved one, and we are sincerely grateful for the trust you have placed in our team.

In accordance with legal requirements, medical records will be securely stored and remain accessible through July 1, 2032. You have the right to request a copy of your records from the past seven years. If you need a copy, we recommend first contacting your primary care or referring provider. You may also request records directly from our practice by submitting a written request to the mailing address below or through our website.

Please note that as of July 31, 2025, our phone number and email address will be discontinued.
